Patient Consent to Telehealth and Clinical Assessment

This consent is a separate document from the Terms and Conditions and the Privacy Policy. You are asked to accept it before your consultation because a telehealth consultation involves choices that are yours to make, and a record of that choice protects you as much as it protects the Clinic.

Where this document refers to the Clinic, it means Menopause Medical Aesthetics Pty Ltd (ACN 701 275 783), trading as Menopause Medical Aesthetics.

1. WHAT YOU ARE CONSENTING TO

1.1 You consent to receiving a clinical assessment and, where clinically appropriate, ongoing clinical care from the Clinic by telehealth rather than in person.

1.2 Telehealth means the consultation is conducted by video, audio or secure messaging, and that your clinician will assess you without physically examining you.

1.3 Accepting this consent does not commit you to any treatment. You may decline any recommendation at any time.

1.4 This consent is not consent to marketing, to the use of your images or story, or to research or artificial-intelligence development. Those are separate, optional choices described in the Privacy Policy, and declining them does not affect your care.

2. THE LIMITS OF A TELEHEALTH ASSESSMENT

2.1 A clinician who cannot physically examine you is working with less information than one who can. Some conditions cannot be safely assessed remotely at all.

2.2 Your clinician may decide that a remote assessment is not adequate for your circumstances, and may ask you to be seen in person, to obtain tests, or to consult another practitioner.

2.3 Technology can fail. A consultation may be interrupted, rescheduled or continued by another means.

2.4 You understand that a decision not to treat, or to refer you elsewhere, is a legitimate clinical outcome of the consultation.

3. NO GUARANTEE OF A PRESCRIPTION

3.1 Paying a consultation fee pays for the clinician's time and assessment. It does not purchase a prescription, a medicine, or any particular outcome.

3.2 Your clinician exercises independent clinical judgement and will prescribe only where it is clinically appropriate to do so.

3.3 Enrolling in an ongoing clinical-care program does not create an entitlement to be prescribed anything.

4. WHAT YOU AGREE TO DO

4.1 You will give accurate and complete information about your health, including your medical history, current and previous medicines, allergies, sensitivities and any pregnancy or breastfeeding.

4.2 You understand that an assessment based on incomplete or inaccurate information may be unsafe, and that this is the single most important thing you control in a remote consultation.

4.3 You will take part from a private location where you can speak freely, and you will tell your clinician who else is present.

4.4 You will not record the consultation without the express agreement of everyone taking part.

4.5 You will tell the Clinic promptly about any adverse reaction or unexpected change in your condition.

5. CLINICAL PHOTOGRAPHS

5.1 You may be asked to provide photographs where they are reasonably necessary to assess or monitor your skin. The purpose will be explained to you before you are asked.

5.2 Photographs you provide for clinical purposes form part of your health record, and access to them is limited to people with a clinical, operational or legal need.

5.3 Your photographs will not be used for marketing, testimonials, social media, publication or model training without your separate and specific consent. Declining that has no effect on your care.

6. YOUR RECORDS AND YOUR PRIVACY

6.1 The Clinic creates and keeps a health record of your care, and handles it in accordance with the Privacy Policy.

6.2 Consultations are not recorded by default. If recording or transcription is ever proposed, you will be told separately and asked separately.

6.3 Skin by MenoWell is a separate education and referral business. It is not your treating clinic and does not receive your clinical records because you were referred by it.

7. PRESCRIPTIONS AND YOUR CHOICE OF PHARMACY

7.1 If you are prescribed a medicine, the prescription belongs to you and you may have it dispensed by any pharmacy you choose.

7.2 The Clinic may offer to arrange ordering and delivery through a named pharmacy as a convenience. That is optional, it is a separate choice from your clinical care, and declining it does not affect your treatment.

7.3 A pharmacy is an independent business. It is responsible for dispensing, supply, delivery, its own charges and its own refunds, and it holds its own patient obligations.

7.4 Where you ask for that convenience, the Clinic will disclose to the pharmacy only what is reasonably necessary to dispense and deliver your item.

8. FEES AND ONGOING CARE

8.1 Clinic fees relate to clinical assessment, treatment planning, monitoring and review. They are separate from the cost of any medicine or item supplied by a pharmacy.

8.2 Where you enrol in an ongoing clinical-care program, you are authorising a recurring charge. What you will be charged, by whom, and when, is shown to you before you confirm, and the arrangement is set out in the Terms and Conditions.

8.3 Where you have also asked for pharmacy ordering, the pharmacy charges you separately in its own name. Two businesses will appear on your statement because two businesses are supplying you.

8.4 You can cancel ongoing care as described in the cancellation and booking policy in the Terms and Conditions.

9. WITHDRAWING THIS CONSENT

9.1 You may withdraw this consent at any time by contacting the Clinic at [email protected]. Withdrawal takes effect from the time it is received.

9.2 Withdrawing consent stops further telehealth care. It does not undo care already provided, and it does not remove records the Clinic is required by law to keep.

9.3 Withdrawing this consent is not the same as cancelling a payment arrangement. If you also want recurring charges to stop, say so, and see the cancellation and booking policy in the Terms and Conditions.

10. EMERGENCIES

10.1 This service is not an emergency service and is not monitored continuously.

10.2 If you are experiencing a medical emergency, call 000 or attend your nearest emergency department. Do not wait for a reply from the Clinic.
